Intervention Services Case Manager

Positive Alternatives is accepting resumes for the position of Case Manager in our Intervention Services Program. There are multiple positions available in Menomonie & Amery. At least one position is full time and multiple part time positions are available.
Intervention Services is a community based program that provides mentoring, supervised visitation, and day supervision to youth, young adults, and families.
The primary duty of Intervention Services Case Manager is to manage a case load of youth and families in each of these programs. The Case Manager facilitates the programs with the youth and families and also develops and implements treatment plans, serves as a primary contact for social workers, parents, and other treatment team members, and completes documentation for programming.
Ideal candidates will be positive, highly motivated, organized, confident, and highly skilled in developing strong treatment/goal centered relationships with clients. To be successful, a candidate will need excellent written and verbal communication skills and be a strong independent worker.
Applicants must be at least 21 years old, hold a valid driver’s license and a driving record free from violations. A minimum of a bachelor’s degree in a human services related field preferred, but may not be required.
These positions will primarily be scheduled with daytime and evening hours Monday through Friday, although some flexibility will be required to accommodate for the needs of the specific youth on the case load, and the scheduling needs of Day Supervision. Some availability may be needed on weekends.
The full time positions are 40 hour per week with a starting pay of $14/hour plus benefits. Benefits include: Medical Insurance, paid holidays and earned personal and vacation time. Also available are dental insurance, life insurance, disability insurance, dependent care, flex spending, and a retirement investment option.
The part time positions are up to 30 hours per week, with a starting pay of $11/hour.
Both the part time and full time positions have the possibility of regular annual increases following successful completion of the initial six month probationary period, the supervisor’s recommendation, and approval from the executive director.
