Associate Case Manager I

Positive Alternatives- River Falls is accepting resumes for the position of Associate Case Manager I.

The primary duty of the Associate Case Manager will be to case manage the youth by developing and implementing treatment plans, and will serve as a primary contact for social workers, parents, and other treatment team members. The Case Manager will accompany the youth to court proceedings, submit required documentation to the court, and testify in court when required. The Case Manager will be responsible to identify the needs of the youth through assessment and gathering a history prior to and also upon intake. The Case Manager will utilize community resources to set up services for the youth that are appropriate, secure transportation, and will provide resident support and supervision during these scheduled appointments. The Case Manager will make monthly recommendations to the treatment teams in written reports regarding placement and discharge dates. Upon discharge, the Case Manager will provide Aftercare Services to the youth transitioning out of the group home for up to six months of time to assist the youth as well as the family.

The Associate Case Manager will also organize, plan, and run 10 week sessions of therapeutic groups for the youth in the home. Additionally, the Case Manager will assist the group home managers in training of all new staff and serve as a very important solution focused liaison, between direct care staff and management. Other job duties assigned by location and supervisor may include but are not limited to, medication duties, menu planning, clerical duties, and direct care.

Ideal candidates will be energetic, positive, highly motivated, assertive, organized, confident, and highly skilled in developing strong and appropriate treatment centered relationships with the youth we serve as well as have the ability to communicate respectfully and effectively with both family members and other collateral professionals including court personnel and law enforcement. To be successful, a candidate will need excellent written and verbal communication skills and a good working skill set regarding computers and technology.

Applicants must be at least 21 years old, hold a valid driver’s license and a driving record free from violations. Applicants must also have 2 years of experience working with this population or an equivalent. A bachelor’s degree in a human services related field desired.

This position will primarily be scheduled with hours from 11a-7pm Monday through Friday, although some flexibility will be required, to accommodate for the needs of the specific youth on the case load, and the scheduling needs of the group home. Some availability may be needed on weekends and evenings. This position is a full time 40 hour per week position. Benefits include: Medical Insurance, paid holidays and earned personal and vacation time. Starting pay for this position will be $14.50 per hour with the possibility of regular annual increases following successful completion of the initial six month probationary period, the supervisor’s recommendation, and approval from the executive director.
